Licensed Practical Nurse

In collaboration with the Nurse Manager, the LPN applies basic nursing skills to the care of persons with chemical dependency addiction to opioids.
Essential Functions And Responsibilities Of The LPN Position
Demonstrates respect for resident’s rights by: Maintaining strict confidentiality of patient information, responding to their right to know, and acting as a patient advocate.
Demonstrates effective assessment skills that identify common physiological and psychological needs, perception and expectations of patient in regards to immediate addiction/treatment care.
Is alert to problems and identifies needs or changes in patient condition and initiates prompt reporting of such to the Nurse Manager or Director of Nursing.
Documents data and scans information into our electronic portal for accurate record keeping.
Participates and complies in meeting both internal & external regulatory standards.
Maintain resident medication records in accordance with Northstar policy requirements, specifically control of drugs on Northstar premises.
Assess critical care needs of residents and provide recommendations for referral to our staff.
Receiving/documenting resident’s calls regarding medication needs or concerns.
Coordinating pharmacy faxes for medication management.
Patient education regarding medications, symptoms, side-effects and labs
Help complete authorization requests.
Complies with regulatory requirements, patient safety requirements, infection control practices, and other established practice standards, policies and procedures.
Minimum Qualifications Required:
Must have two years of freedom from chemical use problems and 18 years of age
Must meet Rule 31 staff qualification requirements as outlined in MINNESOTA RULES 9530.6450
Must pass a State of Minnesota Background Check
Must possess a current Minnesota Driver’s License with acceptable driving record
CPR Certification or attain within 30 days
Current LPN state license
One-year minimum experience in a residential addiction or mental health acute care setting
